His death certificate says he died in Myslowitz. His grave stone in Myslowitz Jewish cemetery appears to show a birth date of 24 April but the year is unclear. The date of death appears to be 24 Kislev 5658 (19.12.1897), agreeing with his death certificate. It names his father rabbi YTZHK Ha Cohn.
